Jubiläum. Der Verband Physician Associates Switzerland feiert am 2. April 2023 sein einjähriges
Bestehen. Er wurdemit dem Ziel gegründet, diesem neuen Beruf eine Stimme zu verleihen, der rund
100 aktive Berufsangehörige zählt undjährlich um 48 Absolventinnen und Absolventen wächst.

Im Ausland, vor allem in den USA und den Niederlanden, gibt es sie schon lange: Physician Assistants (PA). Ihre Aufgabe ist es vor allem, die Ärzte zu entlasten. Bisher werden sie vor allem im stationären Sektor eingesetzt. Doch auch im fachärztlichen ambulanten Bereich sind sie angesichts des Fachkräftemangels auf dem Vormarsch. Im hausärztlichen Bereich sind die PAs aber noch eine Rarität. Die „Hausärzte im Spritzenhaus“ im baden-württembergischen Baiersbronn beschäftigen derzeit gleich zwei PA-Studierende. Dr. Wolfgang von Meißner ist einer der Hausärzte in der großen Gemeinschaftspraxis und offizieller PA-Beauftragter von MEDI. Der änd sprach mit ihm über die Potentiale von Physician Assistant in der hausärztlichen Versorgung und warum sich derzeit nur große Einheiten diese Unterstützung leisten können, obwohl doch gerade Einzelpraxen sie so dringend bräuchten.

Abstract
Introduction: Physician Assistant (PA) have been deployed to increase the capacity of a team, supporting continuity and medical cover. The goal of this study was to assess the implementation of PAs on continuity of surgical rounds, on the collaboration of nurses and physicians and on support of administrative work.

Methods: This cross-sectional survey was performed on nurses and physicians who work full-time at a surgical ward in a Swiss reference center. PAs were introduced in our institution in 2019. Participants answered a self-developed questionnaire 6 and 12months after the implementation of PAs. Administrative work, teamwork, improvement of workfow, and training of physicians has been assessed. Participants answered questions on a 5-point Likert scale and were stratifed according to profession (nurse, physician).

Results: Participants (n =53) reported a positive efect on the regular conduct of rounds (2.9, SD 1.1 points after 6weeks and 3.5, SD 1.1 points after 12weeks, p =0.05). A signifcant improvement of nurse-doctor collaboration has been reported (3.6, SD 1.0 and 4.2, SD 0.8, p=0.05). Nurses (n =28, 52.8%) reported the that PAs are integrated in the physicians team rather than the nurses team (4.0, SD 0.0 points and 4.4, SD 0.7 points, p =0.266) and a signifcant benefcial efect on the surgical clinic (3.7, SD 1.0 points and 4.4, SD 0.8 points, p =0.043). Improved overall management of surgical cases was reported by the physicians (n =25, 47.2%) (4.8, SD 0.4 and 4.3, SD 0.6, p =0.046).

Conclusion: The implementation of PA has improved the collaboration of physicians and nurses substantially. Continuity of rounds has improved and the administrative workload for residents decreased substantially. Overall, the implementation of PA was reported to be benefcial for the surgical clinic.

Spätestens mit den Extra-Belastungen der Pandemie ist deutlich geworden: Hausärzte müssen mit ihren Praxisteams zu einer neuen Arbeitsteilung kommen. Der Vorstand des Hausärzteverbands Baden-Württemberg will mit dem HÄPI-Konzept Patienten, Praxen und Kassen glücklich machen – beispielsweise in einer renovierten HZV.

Ob Krankenwagen, Kardiologie-Ambulatorium oder Militär – sie alle haben den Wert von Gesundheitsfachpersonen mit erweiterten Kompetenzen erkannt. Mit dem neu gegründeten Berufsverband und der Weiterbildung an der ZHAW hat sich das Berufsbild der Physician Associates / Klinischen Fachspezialisten weiter etabliert, wie sich an einem Anlass an der ZHAW gezeigt hat.

Am 24. Juni wird der Berufsverband Physician Associate Switzerland PAS gegründet. Wo steht der neue Beruf Physician Associate heute, 5 Jahre nach der Einführung der ersten Weiterbildung in der Schweiz? Anita Manser, Leiterin der Weiterbildung am ZHAW-Institut für Public Health, gibt Auskunft.

Die «Physician Associates Switzerland» (PAS) steht kurz vor der Gründung. Es sei ein weiterer Meilenstein auf dem Weg zur Akzeptanz der klinischen Fachspezialisten.

Der Fachkräftemangel stellt insbesondere die Versorgung chronisch Kranker und multimorbider Patienten vor grosse Herausforderungen. Ein zentraler Lösungsansatz ist die Integration neuer Berufsgruppen. Auf dem Papier stehen die Konzepte für diese neuen Gesundheitsberufe bereits seit einiger Zeit. Doch wie weit ist die
Integration im Alltag fortgeschritten, und welche Hindernisse verlangsamen die Prozesse? Wir haben bei drei Gesundheitsprofis nachgefragt und sie aus ihrem Alltag berichten lassen.

ABSTRACT

Background: Significant primary care provider (PCP) shortage exists in the United States. Expanding the scope of practice for nurse practitioners (NPs) and physician assistants (PAs) can help alleviate this shortage. The Department of Veterans’ Affairs (VA) has been a pioneer in expanding the role of NPs and PAs in primary caregiving.

Purpose: This study evaluated the health care costs associated with VA patients cared for by NPs and PAs versus primary care physicians (physicians).

Methods: A retrospective data analysis using two separate cohorts of VA patients, one with diabetes and the other with cardiovascular disease (CVD), was performed. The associations between PCP type and health care costs were analyzed using ordinary least square regressions with logarithmically transformed costs.

Results: The analyses estimated 12% to 13% (US dollars [USD] 2,626) and 4% to 5% (USD 924) higher costs for patients assigned to physicians as compared with those assigned to NPs and PAs, after adjusting for baseline patient sociodemographics and disease burden, in the diabetes and CVD cohort, respectively. Given the average patient population size of a VA medical center, these cost differences amount to a total difference of USD 14 million/year per center and USD 5 million/year per center for diabetic and CVD patients, respectively.

Implications for practice: This study highlights the potential cost savings associated with primary caregiving by NPs and PAs. In light of the PCP shortage, the study supports increased involvement of NPs and PAs in primary caregiving. Future studies examining the reasons for these cost differences by provider type are required to provide more scientific evidence for regulatory decision making in this area.

Physician Associate (im Folgenden kurz: PA oder klinische/-r Fachspezialist/-in genannt) ist ein Beruf, der in angelsächsischen Ländern seit über 40 Jahren integraler Bestandteil des Versorgungssystems ist, sich in den Niederlanden in den letzten 15 Jahren etabliert hat und seit einigen Jahren auch im deutschen Gesundheitswesen Fuss fasst. Bereits in der swiss-knife-Ausgabe Dezember 2015 wurde über die Entwicklung berichtet (Clinical Nurse – ein neues Berufsbild).

The physician assistant or physician associate (PA) profession is being adopted in many countries. At a time of improved communication and international exchange of educational methods, no central repository of PA numbers exists. The authors set out to consolidate basic information on PAs. The purpose of the project was to support efforts underway that include the global development of PAs. The prevalence of PAs in each country was obtained using an informant methodology strategy and supplemented with reports and internet validation. Eighteen countries have a PA (similar healthcare professionals with different titles were not included), for an estimated total of more than 132,000 clinically active PAs and 366 training programs. In most countries, PA expansion was reported as being underway.

Seit nun genau zwei Jahren gibt es in der Schulthess Klinik die Funktion der Klinischen
Fachspezialistin. Dabei übernimmt eine ursprünglich aus der Pflege stammende
Fachperson verschiedene medizinisch-ärztliche Aufgaben. Doch was heisst dies genau im Klinikalltag? Nachfolgend erfahren Sie mehr über diese Funktion und wie sich diese in
der Schulthess Klinik entwickelt hat.

Im Rahmen des Mandat 14 des BAG-Förderprogramms «Interprofessionalität im Gesundheitswesen» hat die ZHAW (Winterthurer Institut für Gesundheitsökonomie, Institut für Physiotherapie, Fachstelle Interprofessionelle Lehre und Praxis) untersucht, wie Task Shifting aktuell umgesetzt wird und welche Auswirkungen dies hat. Dabei wurde in drei unterschiedlichen Task-Shifting-Modellenzwischen ökonomischen Auswirkungen und Auswirkungen auf die Versorgungsqualität unterschieden. Übergeordnet wurde untersucht, welche Rolle die interprofessionelle Zusammenarbeit (IPZ) dabei einnimmt.

In der Schweiz sind mit der Pflegeexpertin APN und der Medizinischen Praxiskoordinatorin in den letzten Jahren neue Berufsbilder entstanden, welche die Hausärztinnen bei der Betreuung chronisch erkrankter Menschen unterstützen können.
Wir haben untersucht, was die Aufgabenbereiche und Kompetenzen dieser Gesundheitsberufe sind und wo mögliche Unterschiede und Gemeinsamkeiten liegen.

Because of workforce needs and demographic and chronic disease trends, nurse practitioners (NPs) and physician assistants (PAs) are taking a larger role in the primary care of medically complex patients with chronic conditions. Research shows good quality outcomes, but concerns persist that NPs’ and PAs’ care of vulnerable populations could increase care costs compared to the traditional physician-dominated system. We used 2012–13 Veterans Affairs data on a cohort of medically complex patients with diabetes to compare health services use and costs depending on whether the primary care provider was a physician, NP, or PA. Case-mix-adjusted total care costs were 6–7 percent lower for NP and PA patients than for physician Patients, driven by more use of emergency and inpatient services by the latter. We found that use of NPs and PAs as primary care providers for complex patients with diabetes was associated with less use of acute care services and lower total costs.

With an aging population and an increasing number of people living with chronic disease, Canada’s primary care system is in need of change. Healthcare must better incorporate prevention and patient education in the battle against chronic disease. This article explores the growing role of Physician Assistants (PAs) in enhancing access to appropriate care for the chronically ill, using an example of a PA working as part of a family physician practice in Northern Ontario to improve the care of its diabetic patients.

As the US population increases and ages, more patients require care. A reengineered health care system relies on physician assistants and nurse practitioners; however, the extent to which they care for medical conditions is marginally known. We analyzed ambulatory visits by provider type and diagnosis focusing on chronic diseases to identify differences in patients seen by each type of provider. Both physician assistants and nurse practitioners attended 14% of 777 million weighted visits. Overall, diabetes and hypertension accounted for 2% to 4% of visits. The distribution of visits for chronic disease diagnoses appears to be similar for all 3 providers (physicians, nurse practitioners, and physician assistants). These findings may improve organizational efficiency in ambulatory systems.
